Overall this one is more positive for me. It has its strengths but also its weaknesses. There are quite a few glitches and bugs, and at the beginning the game didn't feel intuitive at all to me. However, after downloading the Doctor Mode DLC (which is free), it made the game pretty fun for me. I still don't understand everything, but overall it is enjoyable. However, I highly recommend checking out other reviews and playthroughs to see if the money is worth it for you.
A very realistic and engaging game with amazing mechanics and playability, especially with DLCs and steam workshop add-ons.

A [i]perfectenschlag[i] simulation of a hospital/clinic on every level. The artwork is amazingly satisfying and reminds me of the Simcity4 rendering. And apart from being entertaining this game is surely educational, giving a clear overview of the hospital dynamics.
The only aspect that I found a bit unrealistic is the diseases featuring a lot of pretty rare ones, but there is mod to mitigate this issue.
Love how in depth this game is. Hours of fun. Wish they were still creating dlc but i respect that it is a fully completed game.
This game was good until they added the collapses. now every couple of minutes, I got so many collapses, that doctors were to busy handling them instead of the patients sitting in the waiting room. So if you want to spend your whole time dealing with patients collapsing, this is the game for you. But if you want a game, running a hospital, this is not it.

SUMMARY:
Good game. If you like games like Prison Architect and want something like it, this is the game for you.
Overall it does really well in the 'accuracy' area, most of the medical stuff in game is as accurate as can be. DLCs are fairly priced but by no means required to have fun in the game. Fair amount of micromanagement involved, but, some areas could still be improved. Mods can help but there are very few mods to pick from, especially large ones that try to add entire departments, very few of those available.
Game has multiple .xml files that you can modify to your liking to improve the game, such as tweaking income to have it not be as much of a concern.
The art style might not be very common in games but it works okay. Eventually you get to the point where stuff is blocking the view of other stuff (note: you can use the [ ] keys to rotate the view and see better, only works in 90 degree increments though)
Audio is good but very few tracks. I personally don't have a problem with that, but, if you cannot stand listening to a track over and over again turn off the music.
Overall Rating: 8.5/10
It's good for medical show lovers
A great, deep, comprehensive simulator.
This is an early sim game, its alot of fun. The tutorials are not very informative but I love these types of games, about to tackle the challenges, then I will be done. lol
A great sim of you like to run a hospital.
Better than Two Point Hospital in every aspect.
Having to figure out what they are visiting the hospital for could be a game itself, but they also added a hospital builder on top of it.
Absolutely one of my favorite games with the additional DLC adding so much opportunity to play with any particular angle or objective.

Excellent Hospital Sim. Some kinks and things left to be desired but overall excellent. Hoping for a Number 2!
I really like this game but I fricking hate the building interface. I wish we could freely move the camera like in any building game and not just have pre made view.

Really cool game, great mods that expand gameplay, great DLC at good prices. Really enjoy the ability to both design the hospital but also play as a doctor/follow patients through the hospital.
THE MAIN THEME IS AN ABSOLUTE BANGER 11/10
Very realistic.
I work in a hospital and in my first sandbox game, I had a doctor who could not decide weather to treat for athletes foot or foot fungus and so the patient almost went home with out treatment.
It is exactly the kind of bad decisions a doctor would make.
Thankfully nurses exist to just tell the patient to put anti-fungal cream on the area.
As a medical clinic staff , this game is pretty awesome and very enjoyable for me. Those different kind of medical equipment, different type of medical department and able to build my clinic workplace in this game is amazing.

I like it, but its... hard. I mean kind of. If you're someone who suffers from ADHD like me, planning is going to be a nightmare (I can't even plan a college presentation assignment properly lmao).
When you get used to it it's mostly fine. I can't say I love the UI though. It has the potential to be a little better, easier to use. First two-three hours gave me strong headaches tryna get used to this shit.

I wanted to like this but the gameplay was so complicated that I found it difficult to enjoy. It's hard to justify the price.
The appeal to this game compared to other simulators is that this focuses on realism rather then sillyness often seen in contemporary games. It requires calculated decisions on building a functioning medical centre.
That is also one of it's weaknesses in my opinion, as it takes alot of energy to keep concentrated. Perhaps an hour at the time before I become strained.
Still there is that satisfaction when seeing a functioning hospital play out after so much work.

It's a simply nice and relaxing game... You can do what you want, the visuals are nice. Good way to kill time.
This is one of those games that just grabs you from time to time where you end up playing for hours. It can be a little overwhelming at first, but once you get into the groove, you can easily lose track of time.
Recommend it for lovers of management games and people who are interested in hospitals.
Thoroughly enjoy this game. It's a little fiddly to build sometimes with the way items overlap with each other, and patient transfers don't always work automatically. Thatt being said, the game is still maintained (despite it being 6(?) years on from release), is one of the most realistic hospital management simulators out there, has a solid, small, modding community and scratches a little itch in my brain that most other in the same genre don't.
There's also an element of playing Dr House as you can significantly speed up tricky diagnoses if you're keyed-in with the symptoms and the right tests to do, or you can focus on optimisation, or essentially whatever you feel like doing.
Great Game For Someone That is looking to Manage Something
It's an amazing game. Just a niche market in general. Its very nice seeing the hospital running and seeing patients leave happy makes me feel good and actually less medically anxious too. Thank you dev <3
I would really love to love this game, but unfortunately, you can tell that it was developed in a very rudimentary way. It lacks many quality-of-life features, the interface is very basic, and the building mode feels like I'm playing a city-builder game from the 90s. For example, there’s no wall-snapping, no way to hide elements in front of others and navigating through the poorly designed Interfaces is a pain.
Normally I'm not really bothered by game music but here you have more or less one song, it gets annoying with time.
I had three or four trys for this game over the last years and every time I stop playing after I'm fustrated how tedious everything feels.
Obsessed with the game.
Every time I look around in my library, I stop by this game and think about playing it.
But I'm not that creative when it comes to building a dream hospital.
I work in a hospital, i said the word "quiet" many times and nothing happened.
i don't hate it it's just too realistic and complex for me.
I've played this since it came out and it is still fun! Wish there was a sequel or more DLC's. The devs are sitting on a goldmine.
Honestly, if you get a hold of the games basic controls, and how to make a layout, it's actually a game you can play to sleep. It's really fun and really relaxing.
Oh, and also, I'm in this longer than most intubated patients lol
This game is really fun. It's kinda "the sims", but with an extraordinarily fun system modeling various ailments and hospital workflow. The same game engine supports anything from a small suburban doctor practice (one man operation) to the largest hospitals.
The game can be enjoyed in a lot of different ways. You can start small with a small practice and treat patients, but refer (and not get fully paid) for the patients in need of treatments or diagnostics you cannot provide.
You can download a fully working and optimized hospital and just enjoy following patients and staff as they interact. Or you can download an empty hospital and gradually hire staff for it and deal with various "hidden" mal-hires (like a person being a drunk or skipping work) or simply that you chose a night owl for the day shift so you you can adjust and fine tune the process.
Or you can take direct control of a doctor and "be" that person inside any of the aforementioned options.
And you learn some medicine while doing it.
It`s not an easy game and the interface can be chalenging but once you get it it`s really amazing. The amount of detail, and the overall experience is great. Very addictive
It's an amazing game. I love controlling doctors and patients and building large hospitals. I would love to see another DLC, possibly an oncology DLC. Keep up the good work.
I wanted to like it, but obviously not for very long. Somehow I thought this would be similar to the old Theme Hospital, but it's not. Constructing the hospital reminded me strongly of Visual Basic during my job my training as an electronics technician in the 1990s. And that's not a good memory. The game is very serious. I got a second job training as a nurse and that's probably the second reason why I don't like this game. The structure of German hospitals is different. There's for example usually not a nurse at the reception but an office employee. I know that's not an important aspect. The important aspect is the realism the game forces upon me. If there were fantasy illnesses, I'd have probably liked this game.

I can tell you, this is the most realistic hospital simulation on the market for now, waiting for Project hospital 2, or another hospital sim game to come out because this game have soo many restriction for "sandbox lover"
I recommended it just because no similar game with better realistic feature exist
Cons:
- Small Map, so many player want to make their real life hospital from their workplace but this game map is limited to 96x96 tiles, so annoying, and limiting creativity (and we dont care about the efficiency!)
- You cant change ambulance parking, because its been a default from the map, you only can make "fake parking with no function at all"
- a lot of bugs
- Only a few departement (No pediatric / ob/gyn departement even its a major dept in real life)
- some diseases have some strange diagnostic pathway or treatment, but still tolerable for a game
This is such a fun simulation game. I like the graphics style, the intricate details and the different game modes. I would like to see more games like this, could add more customise options, and a couple different challenges to continue the "playability" long term. Over all, I love this game!

If you’ve ever wondered what it’s like to run a hospital with zero experience, Project Hospital is here to answer that! Expect patients to show up with a cough and leave with a diagnosis list longer than my grocery receipts. The doctors are either genius lifesavers or on lunch break 24/7. Five stars for teaching me that I’d definitely get fired in real life.
