I could play this game endlessly. I love building and designing a hospital, playing it, learning where i could improve design wise and then starting over from scratch.
This is one of my favourite games of all time. I am desperately hoping they make a sequel or enhance the game further with more DLC (Take my money now!!). I have so many hours in this game and I don't regret a single one of them. I am a control freak so I diagnose and treat every single person who comes into my hospital. I have played two point hospital and theme hospital and while I like them, it is the reality in the diagnosis and treatment in this game that works for me. I also love that you choose your level of control...do you focus on design and let AI do the rest or do you do everything (and if you choose to diagnose do you operate at a junior level with lots of hints or do you ramp it up to specialist and fly blind). I love that I can tailor the game to how I play. I love that it has built in objectives (insurance objectives) and events (with a DLC). I love this game full stop. Aspects of the design and build are a little clunky...but nothing that couldn't be addressed in Project Hospital 2 (hint hint developers).
This is a business sim with a course in medicine. I bought it years ago along with all dlcs. I regret sleeping on this one. Alot of fun if you like business sims. It's not like theme hospital and the diseases are based in real life. You can build your clinic/hospital anyway you want with the most room customizations ive ever seen. The tutorial does a good job of teaching you the game i just wish it was part of a campaign.
Insane game and an absolute killer soundtrack! ♥
really fun and realistic. excited to sink more time into this.
Nice game but it's easy to go bankrupt when you expand a hospital. The costs are high.
One of my favorite games. I def recommend main game plus the add-on's. Endless hours of enjoyment with very few cons. My only complaint would be to add more departments to play with. You could of course download new playable departments with mods but I really think they need to add on to this game because there is a ton of potential.
very nice
Developers don’t distinguish between public services and retail.
The underlying logic design is obviously wrong. You can never build a general hospital. It is more like several specialized hospitals in one building, independently operated entities, from laboratories to toilets.
A Great Hospital Sim Game. If you have ever wanted to run a hospital and micromanage the different aspects that come with that then this game will scratch that itch. That being said it is not a perfect representation of running all aspects of a hospital like it real life. That's not necessarily a bad thing though as some of those aspects would be a bit of a drag. Overall I have put lots of hours into this game and if you get it on sale it is a bargain!
Looks like a decent game but some of the text is tiny when running on a humble 720P laptop, even with the "auto-scale UI" interface enabled. I'd recommend Songs Of Syx; Rim-world; or Oxygen Not Included for people on laptops.
Even Bullfrog's Theme Hospital from 1997 worked great in 480P so this is a disappointment as I was looking forward to trying a new management-simulation game for the 2025 Spring Sale.
As I have no interest in playing this title on a full-monitor set-up, I'm refunding and would only buy again at 90%-off to try again ignoring this problem. This game is 3 times more expensive than ONI; and SOS' free demonstration is the full-game so PH's price-tag doesn't seem to reflect the quality of the product as a first-impression.
At first you feel like you don't know what you're doing or why you're playing but after playing through a few levels you get through the knowledge curve and it becomes quite fun to try and micro manage the hospital for the best results. If you make this far the game hooks you and hits hard to stop playing.
so far so good, buld yourself, or have created rooms, GO create and lead a hospital better then most in the world, because the docs and nurses the employees do listen to you, they make mistakes and do not lie. what I want to see more of is PROJECT PRISON IF IT IS ANYTHING LIKE THIS YOU HAVE A GOOD THING GOING.
I want to open an emergency room, all of rooms already have equipment but when I play the game tha patient don't come,. In waiting room , have A note "planning only, not on foundation". But i already put bench and other. Can someone help me
Best Hospitality Simulation
I wish we have project hospital 2, love the game so much.
Absolutely loving it!
The details are amazing! I could watch the staff and patients runaround all day.
Surprisingly deep and fun! - I still come back to this game every now and again.
this is a great game
This game is great.
Good
This game has a steep learning curve but when you get the hang of it it is an addictive delight to play.
Great game overall but needs update for 2025.
We need Project Hospital 2!
Add more:
- employee/staff characteristics/perks
- employee/staff positions (security, chefs, therapists, orderly, HR, etc)
- levels of equipment set up, options, cost.
- more in game specialty departments
Player should more precisely control employee/staff pay, scheduling shifts, breaks, lunch, etc. not just a day and night/shift.
Make things like rooms with a windows, good or bad lighting more functional.
Loved the game so far! I do wish there are more dlcs coming as there are also other departments worth mentioning.
great game if you like to control a lot of things and control management aspects
very feature rich, including that morose hospital feeling
great game
Love it

Very fun! It's really fun building your hospital and taking over patients to diagnose them. I really enjoy the freedom in building the hospital from the ground up! However if you don't like building parts of the hospitals they do offer pre-built hospitals.
P.H. is a relaxing build time for every decorator. With so many items and too much equipment to fit into small rooms and areas, everything should be placed for top efficiency. Once your build is perfect for you and you've hired all the necessary staff -- and all the required departments and rooms have been perfected with your flair, hop into Game Mode, and watch things go! If your layout and planning are successful, keep an eye on all of the sickly coming through your doors, to insure they are getting the proper care and your staff are handling things. Then, dive in deeper and start taking those patients' charts yourself.
P.H. is one of the best builder, architect, planner, designer, manager, games around. To beat all, you get to play doctor too! Anyone in the profession will have a blast. If you love any of the ER/ED and hospital dramas out there, you too will have a fun time running your own medical campus!
(And no, this was not a paid advert. But if you catch me twitching or on the v-tubes, say hi!)
exciting and challenging. A must play for detailed hospital gaming
fun
Love this game.
Such a good time killer, spend hours perfecting the hospital
love the game!! no other words
Good game. Fire music. I like to take over the diagnosis and treatment of the patients. It would be nicer if they added more departments that are free and don't need DLC. Overall pretty good. Don't kill anyone though.
Oxymoron, you should build another project game like this... It's so good.. Maybe Project University?? Or another project project...
If you like to micromanage things, this is great.
me. like. it. ALL.
Very Detailed game much like City Skylines in my opinion but a hospital and all its quirks. I played a good 6 hours straight-ish when I first got it
Realistic, entertaining, and customizable. I truly they come out with more games like this one!
great at management game's level, but need obgyn department though, feel unrealistic without that..
Incomplete translation, UI panel difficult to be adjusted, and the most important, not enough instruction during the tutorial stages.
Nice idea of as a game, very details about hospital in reality, but too details make it difficult to play.
A great in-depth game and once you get a hold of the layout system the game feels amazing. Wish they had continued with the dlc, especially since there are so many departments they could've added such as plastics, pediatrics and others (some of these can be added via mods but the modders can only work with what the devs already implemented)
This game scratched a very niche itch that I didn't know I had. It's very similar to Rimworld; your role is more of a manager giving the Doctors and Technicians the equipment and the means to perform their tasks and then clicking play and watching them go.
Something that it does differently however, is that it provides you the opportunity to take over the care of the patients that come to your hospital and decide what treatments or examinations they should receive. This is optional of course, but still recommended, as your Doctors aren't perfect. After playing for about 40 hours and providing very little direct control over the care of the patients, only 2-3 of the daily 60-70 patients would return the next day with an incorrect diagnosis. So, as long as you don't cheap out on doctors, you shouldn't need to micromanage much.
The game will inform you of almost every notable event throughout each day, but you can customize the notification of each type of event to your liking; including the color and whether it causes a popup that pauses the game or simply appears in the bottom right as a banner.
The menus and building controls took me a few hours to get fully used to, but they're easy enough to figure out. It can be a bit confusing to understand and navigate through the construction of certain rooms; needing to draw out the room type first and then placing the required furniture within it. Some of the pieces require to be placed in certain positions, and some pieces require other specific pieces to be placed in specific areas around them. This part confused me the most, but it makes sense after a bit of play.
It was a treat to be able to see how each department handles its own category of expertise, and how the different departments coexist. It leads to one needing to develop an understanding of the logistics of how a Hospital operates. For example, the Emergency department doesn't just run the clinics, it also runs the trauma rooms that intake ambulance patients. I didn't understand this and ended up initially placing my clinics on the opposite side of the hospital from where my ambulance drop offs were, and my Emergency personnel would have to run across the hospital to reach the trauma rooms each time an ambulance arrived. The Laboratory and Radiology departments both act as separate entities that each other department can refer patients to, meaning they need to be more central in the construction of the hospital. I misunderstood this and had very small Laboratory and Radiology departments, leading to ICU patients slowly dying in their beds as they waited hours for the opportunity to get a CT or MRI scan to reveal their symptoms and complete their diagnosis.
All of this, coupled with having to manage your funds as you attempt to build the Hospital up, can make for a very interesting challenge! You can only make a contract with three insurance companies at a time, each sending a different amount of patients and paying out a different amount per patient. Each company also provides their own specific challenges, each one providing rewards such as money grants, more patients, or even unlocking new departments or equipment to build.
The graphics aren't much, but they don't need to be. This game isn't meant to look stellar, it's a simulation management game. As for performance, the first hospital I built was on the smaller map. I built it all the way up to the maximum level height, had every department squeezed in with the maximum 400 employees hired. I wasn't making money with it, but the game ran smoothly on higher speeds with no visible performance issues! This is probably the most notable part, in my opinion, as the vast majority of simulation management games these days have a very large problem with performance towards the late game due to how many calculations the CPU has to run every second. I'm still running an I9-9900K, and ran into no issues.
All in all, if you're a management game mogul that likes to control things, this game might be the scratch for an itch you didn't know you had. I found it pleasantly surprising and I've enjoyed every second of playing it. Now I'm gonna go try out mods!
A wonderful hospital management simulation. A more serious one than Two Point hospital, this also means that some parts of it might strike too close to home for some, however a better option if realism is important to you. The game doesn't cover all departments you might find in a hospital, but definately covers most of it.
The management goes quite in depth, and the building is grid based which makes it easy to get a hold on
Despite stopping active development of the game, every so often you might get a random bug fix or patch update. I got all the DLC's for this game, plus some mods to add even more different departments, which means i never ran out of expansions that i could plan to specialise parts of it even more.
If you ever get tired of the management part you could also just rename a doctor after yourself, and start manually treating any patients that enter your office or your department.
Big recommend for anybody who likes management games with emergency services or healthcare as themes.
Addictive and realistic. I wish there was a 128x128 map. More DLCs to come!
After coming back to this game several time to replay, I still absolutely love it. It was hard to grasp in the beginning, but this was totally my fault in not starting tutorials first! I've got all the dlcs, I've done all the challenges, acheivements and I still come back to it. It is on of my top favorite games, def my favorite sim game! Oh how I wish they could continue making new content for this game though! There are so many medical aspects out there that they could add to this. Definite recommend!!
